Medication Request

The best and most efficient way to request a Prescription Refill is through your Pharmacy.

If you need a New Prescription and are unable to request it through your pharmacy, don’t worry. You can use the Patient Portal to make your request. It’s a straightforward process that ensures your request reaches us directly. Your provider may need to discuss changes to your medication regimen with you at times.

If your pharmacy does not have your prescription in stock, you will need to identify a different pharmacy. Please contact your insurance company for assistance. The number is on the front or back of your insurance card, on the insurance company website, or you can call other pharmacies in the area. Once you find a pharmacy, please provide the updated pharmacy information by messaging us in the Patient Portal to allow us to fill the prescription.

Paperwork

If you have State Disability Insurance Paperwork or need a specific letter completed by your provider, you must make an appointment to have them review their portion of the paperwork with you and complete it as appropriate. All paperwork must be handed directly to your provider with all appropriate information (e.g., EDD Claim number). We need you, as the patient, to complete all information needed by the provider before the appointment.

If you have urgent paperwork and have not made an appointment to have the paperwork completed, we will make efforts to address the request, if possible, but recognize, as permitted by your insurance company, there may be a $50 charge for a letter, completed outside of a scheduled appointment.

If you need paperwork to be completed outside of a scheduled appointment, please advise us through the patient portal of the emergent timeline.
